I have a lesson plan — in fact, a whole chapter — about taking responsibility and not blaming others in my book Helping Students Motivate Themselves. I also have posted many related resources — see The Best Resources For Helping Students (& The Rest Of Us) Learn The Concept Of Not Blaming Others.
Today’s New York Times has a good article today on this very topic, Secret Ingredient For Success.
Here’s a quote from it:

I appreciate your entry regarding the quote from the Times article. It is not just students who may turn away from the “brutal assessment” that could change their lives. I work in leadership development for a large organization and we encourage the use of 360 degree feedback by managers in order to get as much feedback on performance and interactions as possible. It is not easy for managers (or anyone for that matter) to turn toward the potentially painful comments to change lives for the better. Of course, this potential pain is mitigated when feedback on the 360 is delivered with executive coaching support.
